EARLY START PNCC


What Do We Do?


Early Start Prenatal Care Program assists new and expecting mothers by assessing their current situation and providing support, guidance and information at a time that can be difficult and overwhelming for them and their families. 




We will match our clients with one of our trained and experienced Care Coordinators. Our Care Coordinators then work with the family to assess what areas they most need help with. We help many families find housing and employment as a part of this service. The family and the Care Coordinator then create a plan to address these issues and set goals to meet. Finally, the Care Coordinator provides information about community resources to support the family in meeting the goals established in their individual care plan

At Early Start PNCC helps women that are currently pregnant or recently given birth. We provide resources that will help them cultivate positive familial environments.

The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: Tobacco Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices Perinatal Depression, Family Planning, and Involved Father.

CARE COORDINATOR 

Child Care coordination services includes
• Improving family functioning
• Improving parenting skills and positive parenting outcomes

Care Coordinator
Qualifications
• Knowledge of maternal childcare issues and community resources
• Ability to interface effectively with a diverse population
• Strong documentation skills; excellent at recording case notes
• Strong verbal communication skills
• Strong organizational and time management skills
• Strong computer skills, including familiarity with the internet and Microsoft Office Suite
• Ability to use the internet and complete accurate data entry
• Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties
• Transport self to the office and to off-site locations in order to accomplish job responsibilities, which include but are not limited to meeting with clients at the office or in their homes, delivering items to clients’ homes, and attending meetings, workshops, seminars, etc
• Ability to use various types of standard equipment/supplies, e.g. pens, pencils, paperwork, computer keyboard, computer mouse or trackpad, telephone, etc
• High school or equivalent (Required)
Responsibilities
• Prenatal Care Coordinators assist at-risk pregnant and postpartum women with accessing medical, social, and educational services related to their pregnancy, in accordance with provider policies and procedures
• Conduct assessment meetings with clients in the office or in the community
• Develop and complete an individualized comprehensive care plan, identifying needs, problems, referrals, etc
• Comprehensive Care Plans are updated a minimum of every 60 days
• Perform health education counseling with high-risk pregnant women, to include smoking, drugs, alcohol, and nutritional counseling or be able to refer appropriate resources
• Focus on providing comprehensive care and referrals to meet the client’s needs
• This includes baby items, treatment needs, medical/dental, housing, legal, financial, benefits, food, clothes, and social needs, etc
• Assist clients with obtaining baby supplies
• Assist clients with transportation needs, budgeting, WIC, etc. as needed
• Assist clients in developing a safe and healthy environment for themselves and their children
• Provide effective communication to patients, staff members, other health care professionals, and referral sources
• Attend and actively participate in provider meetings, and meetings with outside services
• Maintain technical competency and remain current in technology and changes in the industry
• Complete and maintain all required paperwork, records, documents, etc
• Follow and comply with all safety and work rules and regulations
